Physicians use ICD to prevent sudden death caused by certain types of cardiac arrhythmias. Your doctor may recommend an ICD in the following situations:

- If you survived an episode of ventricular fibrillation-threatening.
- If you have had episodes of abnormally fast heartbeat (ventricular tachycardia).
- If you have an enlarged heart as a result of heart disease called dilated cardiomyopathy and syncope (unexplained).
- If you have coronary artery disease, your heart pumps a small amount of blood and has a high risk of having an abnormally high heartbeat (ventricular tachycardia).
- If you have a genetic (inherited) of heart disease that can cause sudden death (hypertrophic cardiomyopathy), even in young people.

Preparation

Your doctor will review your medical history and allergies. He or she will ask for a list of your current medications. If you take medicine to prevent blood clots (anticoagulants), your doctor will tell you when to stop taking them. He or she will also give you instructions on when to stop eating and drinking before the procedure.

An implantable cardioverter-defibrillator (ICD) is a medical device that is placed inside the body. An ICD is threatening abnormal heart rhythms of life (called arrhythmias), including ventricular fibrillation, which causes large muscle chambers (ventricles) of the heart beat in uncompressed or pump blood. When this happens, there is no real beats and there is no sufficient blood flow to the brain and to other organs, including the heart. As a result, a person with ventricular fibrillation loses consciousness and can die within minutes.

An ICD consists of two parts. The pulse generator looks like a small box. It is implanted under the skin below the collarbone. The box contains a rechargeable lithium oxide (lasting between five and nine years) and electrical components that analyze the heart’s electrical activity. Connected to the pulse generator are one or more electrodes, which travel to the heart. When the CDI notice an abnormal heart rhythm, a brief electrical impulse runs deep into the heart, correcting the abnormal rhythm. Many people say that the download feels like a blow to the chest, although the type of pain varies.

In addition to “defibrillate” the heart back into a normal rhythm, the CDI also generate electrical impulses softer. These pulses can be adjusted or “marking time” of an artificial heartbeat if the heart develops other types of arrhythmias. For example, CDI pulses may help lower heart rate when a person has ventricular tachycardia, a beat too fast. Impulses from the CDI can accelerate the heartbeat in cases of bradycardia, an abnormally slow heartbeat.

The CDI also has a record of their actions. This record helps your doctor to control how often have arrhythmias and how dangerous they are. It also lets your doctor see how well the CDI.

ICDs should be monitored periodically. No surgery is needed. A special radio transmitter can receive information from the CDI. In addition, CDI can be programmed to improve performance. Adjustments are made with a small instrument like a wand placed near the body.

To avoid unexpected power loss, IDUs have incorporated a warning notifying the physician when the battery is low. The signal comes several months before the battery runs out. Can be detected in the doctor during a routine check of the CDI.

The first ICD in 1980 were quite simple. Just treat ventricular fibrillation and had a pulse generator large enough (about the size of a pack of cigarettes). Its implementation required major surgery to open heart, followed by a long hospitalization. The CDI newer pulse generators have about 1 inch (2.5 centimeters) in diameter and weigh less than 1.4 ounces (40 grams). His pulse generators are inserted under the skin through a small incision and electrodes can be passed through the veins without opening the chest surgery. Because of these improvements, the implementation of the CDI is now classified as minor surgery.

During the past two decades, more than 200,000 have implemented CDI in people worldwide. During this time, the CDI have shown repeatedly to be life-saving devices. Most clinical studies confirm that people with CDI have a much lower risk of sudden death than comparable individuals who received other treatments. In people who have an ICD to correct ventricular fibrillation, the success rate is nearly 100%.
ICDs are implanted in an operating room of a hospital or a cardiac electrophysiology laboratory.

If you are familiar with biotechnologies and microorganisms, then you probably know who Lindsay Rosenwald is. He is the owner of a multi-billion conglomerate of companies from biotechnology and financial area, and also the most suited person to talk with us about the future of those technologies.

Lindsay Rosenwald seems fascinated about nature and what it can give to us. The nature is generous, and we must value what it gives. This doesn’t mean that we must try to modify it completely. There is a certain order in nature, and it must be respected. Such philosophical beliefs are surprising from a person that seems so pragmatic, but he tells us that without those beliefs, you can’t succeed. There are arguments about biotechnologies, and it is normal for a technology that is still in an experimental phase.

The activity of the Lindsay Rosenwald group has the purpose of solving different problems of the humanity. Some of them have a scientific nature, while others are productive problems. Of course, the educational factor can’t be neglected.

The goals of the biotechnological group of Lindsay Rosenwald are clear in the near future: finding the best methods to grow and deposit the microorganisms, stimulating the metabolism, of those organisms to obtain superior quality germs and mushrooms, and also the usage of natural methods to increase the productions. Moreover, those organisms must be raised in symbiosis with some new mushrooms, with an increased interest from the therapeutic medicine for humans. It is perfectly known that some affection find their remedy in some combinations between the mushroom and the plants, and the results are amazing. Moreover, the group is also interested in educating the students, creating a new generation of researchers and specialists, and also a new educational concept.

The research process needs finality, as the people consuming and using those mushrooms must be satisfied with their quality. The growers continue the research process, and if the different parts implied in the process can’t communicate effectively, the whole process is compromised. This is why Lindsay Rosenwald preferred to gather all the stages of the process under a single name, and his company offers an integrated solution, from the research phase to advanced usages of those organisms. Failure is not an option for this group, and Rosenwald still has some decades ahead to find new organisms and mushrooms that will change the life of humankind forever.

Like any surgical procedure requires anesthesia:

- In the hip, knee and ankle is commonly used spinal anesthesia, the anesthetic is inserted in the back, sleeping both legs above the waist but the patient is conscious.

- In the elbow and wrist is commonly used brachial plexus anesthesia: anesthetic is introduced in the armpit, which numbs the arm operate.

- In the shoulder should be used with general anesthesia.

Practice is usually two (sometimes three) incisions or cuts in the skin, less than a centimeter each, which we introduce:

- A metal tube with an optical system that illuminates and magnifies the inside of the joint, seeing the image on a television monitor.

- A series of hand grippers and tools smoothed tubular motor, for working in the joint.

- To check out the inside of the joint should be filled with sterile serum pressure, which makes the effect of inflating a balloon. What happens after arthroscopy?

Usually the pain after surgery is minimal and the patient can enter outpatient surgery regime, ie it operates in the morning and afternoon can return home. Only in cases of shoulder pain friction can be significant and require one or two days of hospital admission.

The patient must move the joint from day one. After knee arthroscopy does not usually indicate crutches to walk. Yes we often recommend to the ankles. In shoulder arthroscopy, elbow and wrist may not be gaining weight for several weeks at least.

Complications are minimal and usually due to hematomas (bleeding wound to the fat we have under the skin) or to the fluid introduced under pressure, residual joint swelling for a few days. Rarely the swelling compresses the nerves that pass near the joint, causing loss of sensation in the skin or lack of strength. Infection is rare because during arthroscopy fluid pressure that continuously washes the joint.

Arthroscopy is a surgical technique that allows direct view of the interior of a joint and work within it without opening it.

What use is arthroscopy?

It is commonly used for knee problems, shoulder, ankle, wrist, elbow and, less frequently, hip. It has very few side effects compared with traditional surgery.

Be aware that the tests called image, such as magnetic resonance imaging or computed tomography, in many cases are not able to see joint injuries. Arthroscopy is useful when these tests fail, but the problems detected are only those that occur within the joint:

- Injuries to the articular cartilage, forming craters may be eroded (chondropathies) that rub against healthy cartilage, wear out, or pieces can break off the crater, producing loose bodies floating in the fluid that bathes the joint and locked when they change site.- Injury Cover: called synovial capsule which surrounds the joint, and that may be inflamed (synovitis) leading to excess fluid in the joint (effusion), or hardened by scarring (adhesions) coming to be true in this case strings blocking the movement and cause pain.

- Torn ligaments (ligaments are like ropes that connect bone to bone) caused instability: use of the joint. The bones move abnormally with each other to trigger the onset of osteoarthritis.

- Knee: specifically detects the knee pads lesions, called menisci, which when broken can cause pain, or block the movement of the joint.

- Shoulder: shoulder specifically detects the state of the barrier (labrum) prevents the humeral head out of place (dislocated shoulder instability).

Usually the problems detected can also treat:

- In cases of damaged cartilage can smooth the edges of erosion, thereby reducing friction and thereby the pain and stroke. And if there are loose bodies removed to the outside.

- In cases of synovial lesions can smooth growths that form in synovitis and remove the adhesions.

- With arthroscopy can not repair a ligament but in some cases monitor some of the tasks performed by the surgeon to reconstruct it, which requires a minor surgical wound.

- Today is the best surgical technique that can be offered for meniscal injuries, with some exceptions.

In the shoulder allows two specific actions:

- Repair the impeller provided has not had more than three dislocations.

- Currently the best surgical technique to eliminate the friction between tendons and the roof of the shoulder bone, causes a high percentage of pain in the area. If these tendon ruptures occur in some cases can be repaired by arthroscopy.

What else could it be?

Most of the bone pain, even in a patient with a past history of cancer, not due to bone metastases. Most often, the pain may be due to:

- Osteoarthritis

- Arthritis

- Stress Fractures

- Osteoporosis

- Other bone diseases, including metabolic

And sometimes a primary bone tumor occurs in a patient who has suffered other types of cancer.

What can the doctor do?

Bone metastases are treated. The goals of treatment are to relieve pain, prevent fractures or treat that have occurred already, and improve the quality of life for patients, restoring the level of activity that was before metastasis. Treatment includes several aspects:Medical treatment

- Chemotherapy or hormone therapy appropriate for each specific type of cancer

- Treatment of pain with analgesics

- Bisphosphonates, drugs that seem new use to prevent symptoms and complications resulting from metastases, used especially in osteolytic lesions.

Radiation therapy kills cancer cells and relieves pain

- External

- With Samarium radiopharmaceuticals such as strontium or

Surgical

Indicated when there is risk of fracture or when it has already occurred. It is especially indicated in painful metastases involving the lower limbs.

Orthopaedic treatment

With orthopedic braces or splints which allow the limbs to move without pain.

The type of treatment depends on the patient’s general condition, life expectancy, extent of metastatic disease and cancer. Each case will require a specific tatamiento.

What can you do?

If you have had cancer and metastasis has not, follow the advice of your doctor, go to check with your oncologist and reporting the possible occurrence of symptoms as soon as possible.

If you have been diagnosed with bone metastases, besides following the treatment to avoid activities that involve too much effort for your bones. Although daily exercise is considered healthy in general, you should ask your doctor about the advisability of doing to you.

How is living with bone metastases?

They occur generally in patients with advanced cancer or who have relapsed. The appearance of bone metastases in a patient with lung cancer is associated with a half-life of less than 6 months. However, when they occur in thyroid cancer and breast cancer, the prognosis is not so unhappy, and patients may live for several years.

Compared with patients who have metastases in other organs other than bone, patients with bone metastases have a better prognosis.

Vary depending on the patient. Some people have no symptoms, sometimes you discover a relapse of cancer because bone metastasis. In some cases, people go to the doctor with complaints caused by a metastasis and primary cancer has not been diagnosed yet. The symptoms most commonly associated with are:

- Pain is usually the first thing to note. It is usually a localized pain that increases with time and depends on the site of metastasis. When it affects the spine, is usually worse at night and rest, but the metastases in the lower limbs often cause pain that worsens with activity and improves with rest.

- Anemia: Because metastasis replaces the normal marrow. Symptoms of anemia include fatigue, weakness, pale skin and mucous membranes.- Hypercalcemia: or too much calcium, because bones are physiological calcium deposits that are released upon destruction. Symptoms of hypercalcemia are nonspecific: nausea, thirst, fatigue, exhaustion and finally, altered mental status.

- Pathological fractures, due to the cancer cells that replace the bone does not have the properties they support.

- Compression of the spinal cord: this happens mainly when metastases are found in the vertebrae and they collapse, causing compression of the spinal cord in the spinal canal. The symptoms are so severe weakness or numbness in the legs, loss of sphincter control or severe pain in the back.

How are they diagnosed?

Need to do a careful review of the patient’s medical history in addition to their family history and a thorough examination. This information is attached to that obtained by laboratory tests such as:

- Blood tests: Blood tests detect symptoms such as anemia or hypercalcemia. Sometimes, there are compounds that do not normally exist in blood and whose detection for the diagnosis of a particular type of cancer or the control of relapse. Are tumor markers, eg CA-125, PSA, CEA and beta2-microglobulin.

- X-rays: X-rays are useful for diagnosing metastases and to determine the risk of fracture.

- Scan or bone scan: a highly sensitive test that detects the presence of metastases even in normal X-rays and has the advantage of covering the entire skeleton in the scan.

- CT: MRI scans are indicated only in exceptional cases, when X-rays and scans do not give a clear diagnosis.

- Biopsy: metastasis biopsy is indicated in cases where there is a single injury, after a long period of complete remission of the cancer, to distinguish it from a primary bone tumor.

All these tests not only help determine the extent of the disease but also to monitor the success of therapy is established.

They are clusters of cells in bone cancer from another organ in which it arose another malignant tumor.

Cancer of the organ from which it is called primary, and is most often of the prostate, breast, lung, thyroid and kidney, although almost any cancer can give them. From here, the tumor cells enter the bloodstream and appear in the bones, where they have the capacity to continue growing.

The preferred sites for metastases are in order of frequency, liver, lung and bone.

Bone metastases are more common than primary bone tumors (25:1) and occur mostly in people over 50 years. The bones in the most frequently settle metastases are the vertebrae, ribs, skull, femur and pelvis. Although sometimes are unique, most commonly appear in multiple places.
Why is bone metastasis?

Occur because the primary cancer has invaded the neighboring vessels and tumor cells circulating in the blood. Bones have many small vessels that hold the cells in its path and provide a suitable environment for growth.

Most bone metastases destroy the bone and are seen in radiographs as areas darker than the surrounding bone, which are called osteolytic. Some, such as prostate cancer and some breast cancers form bone and denser are called osteoblasts. There are also mixed types.

Because every time we can extend the life of cancer patients for longer, the chances of metastasis increases.

Very rarely a rupture arterial limb becoming pale, cold, pulseless, and the patient suffers pain throughout her violent. It is sometimes manifested by a sudden and significant bruising in the armpit. Require urgent surgery in a department of vascular surgery.

Rarely injured nerves, and affects more circumflex, which reflects the sensitivity of the skin of the shoulder and deltoid muscle strength. It is much more rare brachial plexus injury. Usually recover well but it takes several months.

It is frequent, especially in middle-aged patients, injury to the rotator cuff around the shoulder joint. Causes a conflict of subacromial space (rubbing against the tunnel that surrounds him) or limited strength and range of motion of the arm when it breaks. What develops a dislocated shoulder?

- In young patients it is common for non-operated anterior dislocation recurrence, due to vigorous physical activity.

- In older patients usually does not recur but pose problems of the rotator cuff and capsular stiffness that limits movement of the shoulder.

Posterior dislocations can recur (come back), if the patient goes into convulsions.
